Alumni Entity Convention Accreditation Report

The National Bylaws establish the requirements necessary for an alumni entity to be recognized by the Fraternity and, therefore, to be eligible to have a voting delegate at each Annual Convention. To seek recognition as your Alpha’s alumni entity, both the Annual Report and the Accreditation Report must be completed. Please return the Annual Report to the CO along with the required attachments, complete the Accreditation Report below, and submit $275.00 for the Convention fee. These reports and attachments can be mailed or faxed, but are preferred to be sent via email as a .pdf to the Central Office. Convention fees may also be paid via credit card at the online store or during your Convention registration process.

To ensure that there is ample time to review these reports and to ask for any needed additional information, please submit this form, required attachments, and the following form no later than one month prior to the Convention.

If your Alpha maintains a Lodge or other Chi Psi property, please also complete the Annual Lodge Report so we can stay up-to-date on the current status of your Lodge or property.

NOTE: There are three separate forms below - one for Advisory Boards, one for Corporations, and one for Regional Associations/Alumni Clubs. Please ONLY fill out the appropriate form for your entity.
