Forms for the Central Office
The following forms should be submitted to the Central Office using the online forms contained within this section. Use the links below or the drop down menu above to access each form.
Pledge Roster - This form should be submitted immediately following the formalization of your Alpha's pledge class. Pledge fees should be sumitted concurrently with this form
Membership Form - Immediately upon pledging, Pledges should register for this site, log in, and fill out this form. This information will be used to create a detailed profile in our database and prepare a badge order.
Initiation Roster - This form is due BEFORE initiation. Initiation fees should be sumitted concurrently with this form and must be received BEFORE initiation. Following initiation, Pledges' profiles will be assigned active status by a site administrator. Badges will not be ordered until the membership form, initiation roster, and payment are received.
Officer Update Form - This form should be completed immediately following the election of new officers.
Alpha Calendar - Use this form at least once a semester to advise the Central Office of important dates in your Alpha's calendar.
Member Change in Status Form - Use this form to inform the Central Office of any change in active status (i.e. to dormant active status). These changes will affect invoicing for dues, insurance, and convention fees, so please be diligent.
